The COPE project developed a new data collection system for Community Health Workers (CHWs) to streamline their efforts in addressing health disparities. The system includes features like needs assessments, progress tracking, and resource documentation. Pilot tests showed positive feedback from users, with over 55,000 documented encounters and 5079 clients. The system improves client engagement, enrollment processes, and identification of resources. It serves as a model for community-based documentation systems and can be adapted for other settings.
