This study looked at how community health workers (CHWs) can help identify and address social risks in primary care. Researchers worked with two health centers in Arizona to map their processes for screening, referring, and following up on social risks. They found gaps in the system and suggested ways to improve how CHWs are integrated into the process. The study highlights the value of using process mapping to improve care and suggests that better CHW involvement can make social risk screening more effective.
