This study investigates the implementation of a community health worker (CHW)-delivered care coordination program for students with asthma in West Philadelphia schools. Surveys and interviews with school staff highlighted perceived benefits, barriers, and facilitators to program implementation. Challenges included workflow integration and communication, while supportive school administrators and staff were identified as crucial facilitators. The findings provide insights for implementing similar asthma control programs in schools.
