This study investigated the effectiveness of community health workers (CHWs) in referring individuals with elevated blood pressure (BP) to local health resources in Soweto, South Africa. Results showed that CHWs successfully screened a large number of community members and provided referrals to a physical activity (PA) program or local health clinics. Factors influencing referral utilization included proximity to resources, education level, perception of health severity, and barriers like childcare and safety concerns. Overall, the study highlights the potential of CHWs to bridge gaps in accessing healthcare resources for non-communicable disease (NCD) management in under-resourced communities.
