Home visits versus fixed-site care by community health workers and child survival: a cluster-randomized trial, Mali

This study tested if regular home visits by trained community health workers (CHWs) could improve child survival in rural Mali. In a trial from 2017 to 2020, 137 villages were divided into two groups: one received CHW home visits, while the other received care at fixed locations. Both groups had improved health centers and free services. Over three years, under-five mortality rates dropped in both groups, but there was no significant difference between them. The decline in child deaths may have been due to overall improvements in the health system, not the home visits.
