This study explains how NYC Health + Hospitals set up a large Community Health Worker (CHW) program in adult primary care clinics from January 2022 to December 2023, and how they tracked its progress. Key data included details about the CHW workforce, patient demographics, and patient engagement, such as the number of patients contacted and goals achieved. By mid-2023, 97 CHWs and 22 supervisors worked across 17 sites. CHWs mostly served older and African American/Black patients. Out of 13,812 patients contacted, 7,331 were enrolled, with many focusing on goals like primary and specialty care and addressing food insecurity. Clear tracking helped ensure CHWs could effectively engage patients and connect them to services.
