This study describes how NYC Health + Hospitals implemented a large Community Health Worker (CHW) program in adult primary care from January 2022 to December 2023. It also outlines the metrics used to track the program’s progress. Data collected included workforce demographics, patient details, and program outcomes like patient engagement and goal completion. By June 2023, 97 CHWs and 22 supervisors worked across 17 sites. CHWs reached over 13,000 patients, with 7,331 enrolled and 5,044 successfully completing the program. The main goals were improving primary and specialty care engagement and addressing food insecurity. Clear metrics ensured meaningful patient engagement and connection to services.
