This study aimed to understand how community health workers (CHWs) connect clients to resources and create a model for effective CHW referral systems. Through interviews with CHWs, key actions were identified, such as assisting with paperwork and transportation, actively communicating with resource organizations, and following up with clients. Themes emerged related to facilitators and barriers, including cultural/language consistency, coping strategies, and institutional factors. The study highlighted additional actions taken by CHWs beyond established processes to support successful referrals, emphasizing the importance of building trust with clients. Enhancing support for these actions can inform replicable structures for efficient and effective referral processes.
